Job Description

As part of the Telecare Family, Multnomah Crisis and Treatment Center (CATC) is a subacute 16-bed, 24/7 program that serves as a secure short-term crisis program for adults with persistent and serious mental illness, who live in Multnomah County.

Shifts Available: AM 7:00 am - 3:30 pm / PM 3:00 pm - 11:30 pm / NOC 11:00 pm - 7:30 am

On Call; Shift Hours and Days vary as needed

Expected starting wage range $24.50 - $30.27 - Telecare applies geographic differentials to its pay ranges. The pay range assigned to this role will be based on the geographic location from which the role is performed. Starting pay is commensurate with relevant experience above the minimum requirements.

Shift differentials for hourly staff (6% for PM Shift, 10% for Overnight Shift). Weekend Shift differentials for hourly staff (5% for Weekend AM Shift, 11% for Weekend PM Shift, 15% for Weekend Overnight Shift)

POSITION SUMMARY

The Qualified Mental Health Professional (QMHP) provides assessment, treatment planning, and treatment to residents.

ESSENTIAL FUNCTIONS

o Understands the use and recognizes the side-effects of psychotropic medications.

o Provides psychotherapy targeting specific psychological and behavioral problems

o Participates in Close Observation duties

o Assesses risk for suicide, self-harm, and violence

o Evaluates residents for potential risk and takes steps to mitigate identified risk using psychiatric holds.

o Must assist with restraint of members served in the event of assaultive behavior and pass assault crisis/crisis prevention training

Additional Functions:

* Demonstrates the Telecare mission, purpose, values, and beliefs in everyday language and contact with the internal and external stakeholders

* Performs psychosocial interviews and develops written clinical assessments with assigned members served

* Ensures that members served are authorized for services by developing written authorization requests per contract requirements and submitting them to the Payor for approval

* Uses collaborative and motivational interventions to build engagement

* Performs service coordination functions of assessing, planning, linking, monitoring, and advocating for members served

* Ensures documentation of all services in accordance with standards

* Collaborates with the treatment team in coordination of member services

* Educates the member served to prevent health problems and coordinates medical screening and follow-ups

* Encourages and motivates members served regarding work, especially competitive employment and school as achievable goals

* Educates the member served about mental illness, treatment, and recovery and teaches skills for coping with specific symptoms and stress management

* Provides skill-building, coaching, and access to necessary resources to help members served with personal care, safety skills, money management, shopping, household management, social skills, and use of transportation resources

* Assumes professional responsibility for members served well-being

* Responds to crisis situations

* Assure that members served know and understand their legal status and rights; tracks legal status of members served

* Observes, records, and reports members served social, activities of daily living (ADLs), substance abuse, vocational, and psychiatric behavior per treatment plan guidelines

* Completes an individualized and comprehensive service plan, with input from team and family members, including assessing the member served strengths, preferences, and goals as well as educational, substance abuse, housing, vocational, social, financial, and health needs

* Facilitates the development and updating of comprehensive crisis plans for members served

* Provides psycho-educational services to families and significant others to facilitate communication skills and problem solving and enlist the involvement of natural supports to support the recovery of the member served

* Develops community resources and builds positive productive relationships with community partners

* Provides advocacy on members served behalf in an effort to negotiate troublesome system boundaries

Duties and responsibilities may be added, deleted and/or changed at the discretion of management.

QUALIFICATIONS

Required:

* Graduate degree in a behavioral science field, psychology, social work, or recreational, art, or music therapy

* Must have knowledge and ability to use public transportation

* Must be at least 18 years of age

* Must be CPR, Crisis Prevention Institute (CPI), and First Aid certified on date of employment or within 60 days of employment and maintain current certification throughout employment

* All opportunities at Telecare are contingent upon successful completion and receipt of acceptable results of the applicable post-offer physical examination, 2-step PPD test for tuberculosis, acceptable criminal background clearances, excluded party sanctions, and degree or license verification. If the position requires driving, valid driver's license, a motor vehicle clearance and proof of auto insurance is required at time of employment and must be maintained throughout employment. Additional regulatory, contractual or local requirements may apply

Preferred:

* Awareness of Recovery and Rehabilitation philosophy

* Specialized knowledge/experience in vocational, housing, or dual diagnosis services

* Bilingual abilities

SKILLS

* Risk assessment skills for suicide, self-harm, and violence towards others at inpatient facilities

PHYSICAL DEMANDS

The physical demands here are representative of those that must be met by an employee to successfully perform the essential functions of this job.

The employee is occasionally required to sit, bend, squat, kneel, push, pull and lift and carry items weighing 25 pounds or less as well as to frequently stand, walk, reach, twist and do simple and power grasping. The position requires manual repetitions, deviations and dexterity and to occasionally drive. Occasional Hazardous Exposure (Blood Borne Pathogens, hospital waste, Chemicals & Infections).
